I had surgery back in April of 2021. My pre op and everything was performed in hazel crest. I pay my last payment and get everything finalized and find out I’m having survey done in Indiana. Fast forward to the day of survey. I let them know number numbers of times my IV BURNED the numbing process was like balls of fire I fell asleep and woke up in a wheel chair by the FRONT DOOR…. Fast forward to months later having issues with upper abdominal pain. fast forward to today to find out he had negligence against multiple women and even a death, his license was no longer valid to work in Illinois to he open up sho IN A TRAILER to do procedures now.
