Dr. David C. Yao is one of the three founders of Forma Plastic Surgery, where premier plastic surgery meets and exceeds the desires of its patients. Dr. Yao is board certified by the American Board of Plastic Surgery and the American Board of Surgery. He is also a cosmetic fellowship sub-specialized trained with an emphasis on the face, breast, and body. His extensive education and experience allow him to offer his patients the most advanced, cutting-edge techniques in cosmetic plastic surgery. Dr. Yao thrives on the blend of science and art of plastic surgery. His artistic eye and scientific concepts enable him to design unique treatment plans for each patient and provide beautiful, natural-looking results.

Education

Born and raised in Southern California, Dr. Yao attended the University of California, San Diego and received his bachelor’s degree in physiology. He completed his doctor of medicine degree at Medical College of Virginia (Virginia Commonwealth University School of Medicine) in Richmond, VA. After receiving his degree, Dr. Yao relocated to Arizona and attended his general surgery residency at Maricopa Integrated Health Systems. Here, he obtained significant experience in complex surgical patients.

Dr. Yao gained extensive plastic and reconstructive surgery experience with his education at Mayo Clinic in Rochester, MN. As part of a well-rounded plastic surgery education, he received in-depth training in cosmetic, craniofacial, hand, and general reconstructive surgery. Following his Mayo Clinic experience, Dr. Yao completed a prestigious cosmetic plastic surgery fellowship at the Manhattan Eye, Ear, and Throat Hospital (Lenox Hill Hospital/NYU). While there, he had independent privileges and attained significant plastic surgery operative volume. Dr. Yao also worked with world-renowned Park Avenue and Manhattan plastic surgeons, perfecting many of the techniques he applies today. Before returning to Arizona, Dr. Yao held the position of Attending Plastic Surgeon at Mayo Health System. In 2011, Dr. Yao returned to Arizona and joined his fellow colleagues, Dr. Walsh and Dr. Sabeeh, to create Forma Plastic Surgery. The three physicians created a highly respected and well-rounded practice to meet and exceed every individual’s aesthetic goals.

While there, he attained significant plastic surgery volume and perfected many of the techniques he applies today. Dr. Yao believes in a natural-looking aesthetic enhancing each patient’s already existing beauty and yielding long-lasting results.
